1996 Dodge Grand Caravan. What is the procedure to synchronize a new key remote?

No existing remote available. 1996 Dodge Grand Caravan. The basic owners manual procedure requires an existing synchronized remote to perform, and says "contact dealer for instructions" when no existing synchronized remote is available!!!!

It's true - without a working remote the dealer or an automotive locksmith has to program that one in.
